Based on record reviews, the licensee did not comply with the section cited above in 2 out of 2 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees that she and her assistant will renew the required Mandated Reporter Training. Proof of completion shall be submitted to CCLD on or before 10/23/2025.

Based on an interview, the licensee did not comply with the section cited above in one out of one object which poses/posed a potential health, safety or personal rights risk to persons in care. Upon arrival to the home LPA observed a Baby Bedside Sleeper/Bassinet in the dining room an area used by and accessible by children in care.

Resolution: The licensee agrees to provide proof of having removed the Baby Bedside Sleeper/Bassinet from areas inaccessible to children. The licensee agrees that this and any like equipment shall not be used with infants/children in care. The plan of correction shall be submitted to CCLD on or before 9/26/2024.

Based on children's record reviews, the licensee did not comply with the section cited above in one out of five children file reviews the licensee failed to document Infant #5 sleep every 15 minutes as required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to submit one weeks worth of 15 minute sleep documentation for Infant #5. Copies of the documentation shall be submitted to CCLD on or by 4/13/2023.
