This requirement is not met as evidenced by a review of records which revealed 2 children do not have immunization records in file
Resolution: Licensee will have each child's parent obtain a copy of their immunization record. Licensee will submit copies of the children's immunization records to community care licensing by 2/24/26

Resolution: Licensee will have each child's parent read and sign the parent's rights form. Licensee will submit a copy of the parent's rights reeipt to community care licensing by 2/24/26. Failure to comply will result in a $50 civil penalty per child.
