Based on interview and record review, the licensee did not comply with the section cited above in [count] out of 3 out of 3 adult employees which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must provide LPA with proof of CPR certification by POC date for all adults employed with facility who are left alone with children at any time.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 1 infants which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Proof of 15 minute checks conducted for all infants under 24months of age must be provided to LPA by POC date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 years worth of required fire drills which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must provide LPA proof of fire drills conducted within 30 days (maintain once every six month requirment) by POC date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 3 out of 3 adult employees which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must provide LPA with proof of Mandated Reporter Training certification for all adults working with children in the home by POC date.

Based on observationand interview, the licensee did not comply with the section cited above in 1 out of 1 yards which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will make backyard off limits immediately by POC date. Purchase required pool ring, hook and latch and alarm. Proof of purchase must be sent to LPA.

Based on observation and interview, the licensee did not comply with the section cited above in 1 of 1 persons which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee must maintain presence in home at least 80% of day. In the event of Licensee needing to be gone longer, the facility is to shut down for the alotted time. Follow up visit (unplanned) to be conducted to verify presence.

Based on observations, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure they renewed their EMSA pediatric CPR and first aid certification by the expiration date which poses a potential health, safety, or personal rights risk to children in care.

Resolution: Licensee agreed to provide proof of enrollment and/or completed certificate to LPA by 11/04/2023

Based on observations, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure required immunizations were maintained for one staff present which poses a potential health, safety, or personal rights risk to children in care.

Resolution: Licensee agreed to provide proof of required immunizations for assistant to LPA by 11/04/2023

Based on observations, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure mandated reporter trainings were completed by the expiration dates which poses a potential health, safety, or personal rights risk to children in care.

Resolution: Licensee agreed to provide completed certificates for themselves and their assistant to LPA by 11/04/2023
