Based on record review, the licensee did not comply with the section cited above in that the licensee and her assistant have not taken the Mandated Reporter Training since 2018, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and her assistants will take the required training by the POC date of 10/21/21. Licensee will submit proof/copy of training certificates to CCLD by the POC due date of 10/21/21.

Based on record review, the licensee did not comply with the section cited above in that one infant did not have the required documentation (LIC 9227) in the child's file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have parent complete form LIC9227 for child's file by the POC due date of 10/14/21. Licensee will submit proof of completion of form by the POC due date of 10/14/21 via text or email.
