Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have emergency information card available and maintained during the inspection. Licensee was directed to complete and submit proof to Community Care Licensing.

Resolution: Licensee was directed to submit completed form to community care licensing by plan of correction date listed.

Based on record review, the licensee did not comply with the section cited above as neither the licensee or her assistant had proof of completing the Mandated Reporter training. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee and her assistant agree to complete the Mandated Reporter training and have proof of completion available for review by the due date of 7/12/22. A return visit will take place in order to clear this deficiency.

Upon inspection, the LPA observed 5 off limits rooms that were open and accessible to children. The locks were not latched on 3 of the rooms and 2 of the rooms were unable to be locked. There were no spinners either. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee and her assistant agree to take the on line training, "Locks and Inaccessibility Requirements in Child Care" on the CCL website and write a statement of what those requirements are. This statement will be available for review by the due date of 7/12/22. A return visit will take place in order to clear this deficiency.

Based on record review, the licensee did not have proof of MMR or T-Dap available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to get copies of her MMR an T-Dap and have available for review by the due date of 7/12/22. A return visit will take place in order to clear this deficiency.

Based on record review, two out of four files reviewed did not have proof of parent rights notification in the files. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to take the "Record Keeping in Family Child Care" on the CCL website and get all children's files up to date and have available for review by the due date of 7/12/22. A return visit will take place in order to clear this deficiency.
