Licensee's assistant did not have current mandated reporter training completed. Licensee was directed to submit proof to community care licensing.
Resolution: Licensee will have assistant complete training and will submit proof of completion to community care licensing.
Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did no have immunzation records for one child enrolled in her family child care home. Licensee was directed to obtain immunization records and submit proof to community care licensing.
Resolution: Licensee will obtain immunization record and submit proof to community care licensing.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. The licensee could not provide LPA with her current mandated reporter training certificate and the assisant has not completed the mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will provide the LPA with her and her assistant's Mandated Reporter Training by 01/22/24.
Based on record review, the licensee did not comply with the section cited above in that licensee could not provide completed Individual Infant Sleep Plans (LIC 9227s) for two infants in care on this inspection, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit completed LIC 9227s for the two infants in care to CCLD by the POC due date of 03/09/22.
