Based on observation, the licensee did not comply with the section cited above in LPA observed a baby bouncer in the living room. Licensee stated that she had used the bouncer to place infant to sleep. LPA reviewed safe sleep regulations with licensee. LPA did not observe infant sleeping in the bouncer. Licensee removed bouncer and placed it in a room that children do not have access to. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA reviewed with licensee items that are not permitted in a family child care home. Licensee removed bouncer from living and placed it in a off - limits room. Licensee stated that she will not use the bouncer. Licensee stated she will write a statement and submit it to the department.
Based on record review, the licensee did not comply with the section cited above in that LPA observed that licensee did not have records of infant's 15 minute sleep log for child 4. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA reviewed safe sleep regulations with licensee. LPA printed a copy of the in-house 15 minute checklist and provided two copies to the licensee. Licensee stated that she will record two weeks of the 15 minute log for child 4 and any other infants she cares for. Licensee stated that she will submit proof to the department by 03/01/2024.
Based on record reveiw the licensee did not comply with the section cited above in that LPA observed that there was no file for child 3 and child 4, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will search for files, and provide proof to the department by 03/01/2024.
Based on record review, the licensee did not comply with the section cited above in that LPA observed that licensee did not have an updated Children's Roster, which poses/posed a potential health, safety or personal rights risk to persons in care.
