Based on observation and Licensee's admission, the licensee did not comply with the section cited above female adult is not fingerprinted which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA Denisia Jimenez a picture of the Live Scan Form LIC9163 once the female adult goes and gets fingerprinted.

Based on record review, the licensee did not comply with the section cited above that the Licensee has no immunizations for the children enrolled as required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send LPA copies of the the children's immunizations by the plan of correction date of 5/15/23.

Based on record review, the licensee did not comply with the section cited above. Licensee did not provide child's parent or authorized representative with a copy of the notice Family Child Care Home Notification of Parent's Rights, LIC 995 which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. Licensee has no emergency information card as required for the children enrolled which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send LPA copies of the the children's emergency information card by the plan of correction date of 5/15/23.

Based on observation, the licensee did not comply with the section cited above in that the facility smells of pet urine, LPA observed pet feces inside the common areas of the home and floors are dirty throughout the home, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have home cleaned and without odor upon LPA's POC Inspection.

Based on oservation, the licensee did not comply with the section cited above in that Licensee did not have a fire extinguisher that meets licensing requirements, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase a fire extinguisher that meets licensing requirements prior to LPA's POC Inspection.

Based on observation, the licensee did not comply with the section cited above in that cleaning compounds, i.e. Comet, Ajax, window cleaner and bleach were stored in a kitchen cabinet under the sink, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: During this inspection, Licensee immediately removed the items from under the kitchen sink cabinet and placed them in an area inaccessible to children in care.
