Based on record review, the licensee did not comply with the section cited above in count that licensee did not have the individual sleeping plan LIC9227 on file for the two (2) infants she has in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure to complete forms with infants parents. Licensee will submit a copy of both LIC9227 forms to the Department by 05/19/2025. During inspection LPA reviewed safe sleep regulations. Licensee stated she will contact the Department if she had any follow up questions or concerns.
