Based on record review, the licensee did not comply with the section cited above in that Child #4 did not have a record of the 15 minute safe sleep logs on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit proof of 15 minute checks/logs to the Department by POC due date.

Based on observations, the licensee did not comply with the section cited above in that LPA observed Adult #1 in Room #3 hiding under bedsheets on the bed. LPA observed that Adult #1 is not a cleared adult. LPA observed adult clothes and hygiene products in the room. Licensee informed LPA that Adult #2 is Adult #1's spouse and stays in Room #3, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will get both Adult #1 and Adult #2 fingerprints done and provide proof of correction to the Department on 1/10/2025. Licensee will watch video "Background Check Requirements for Caregivers" from the CDSS website. LPA provided link to Licensee. Licensee will watch video and provide a through written statement on what they have learned. Licensee stated that Assistant #1 will aide in translation.

Based on observed, the licensee did not comply with the section cited above in that licensee had installed a new play yard equipement in the backyard and installed a new fence, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will watch "Child Care Reporting Requirements" video from the CDSS website. LPA provided link to Licensee. Licensee will write a through statement on what they have learned and submit it to the department by POC due date. Licensee stated that Assistant #1 will aide in translation.

Based on recrod reveiw, the licensee did not comply with the section cited above in that LPA was informed by licensee that Child #5 and Child #6 were dropped off at day care, on today's date, and do not have file on site, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will watch "Record Keeping in Family Child Care" and write a statement on what they have learned. Licensee stated tha they will have Assistant #1 help with translation. Licensee will submit proof of a complete file for Child #5 and Child #6 by POC due date to the Department.

Based on record review, the licensee did not comply with the section cited above in that, LPA observed that the last Fire Drill conducted was on 09/09/2021 and had not been conducted within the 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will complete a fire drill and submit proof to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above in that, LPA observed in bathroom drawer a pocket knife that had no self latch and was easily opened and accessible to children in care. LPA also observed a screw driver in bathroom drawer accessible to children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed that Licensee removed pocket knife and screw driver. Licensee placed pocket knife and screw driver in garage that is locked and inaccessible to children. Licensee had stated that they will watch video found in www.ccld.childcarevideos.org titled "Locks and Inaccessibility Requirements in Child Care" and provide a written signed and dated statement on what they have learned to CCL on POC due date.

Based on observation, the licensee did not comply with the section cited above in, LPA observed in backyard a hole in the ground with burnt wood covered by a rusty metal lid that is accessbile to children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will cover hole with sand and toss out rusty metal lid. Licensee will provide photos of correction to CCL on POC due date. Licensee stated that children will not play in grass area untill hole is covered, and children will play in gated patio and do not have access to grass area.

Based on record review, the licensee did not comply with the section cited above in that, LPA observed that Licensee, Assistant #1, and Assistant #2 did not update their Mandated Reporter Certificate. Licensee last completed their Mandated Reporter on 2/19/2020. Assistant #1 last completed their Mandated Reporter on 2/17/2020. Assistant #2 does not have a Mandated Reporter Certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she, Assistant #1, and Assistant #2 will complete their Mandated Reporter AB1207 and submit proof to CCL by POC due date. Training can be taken at www.mandatedreporterca.com available in English and Spanish.
