COSIO FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in that LPA observed various items that should be kept out of reach of children in care accessible to children during inspection as stated in the LIC809 Facility Evaluation Report, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed all of the "Keep out of Reach of Children" items and placed them in a inaccessible area. Licensee corrected during inspection.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current FIRE DRILL, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill and provide a copy of the drill to the department. LPA provided licensee with an example sheet of the drill.

Based on record review, the licensee did not comply with the section cited above in that LPA observed that a child under the age of 24 months did not have records of the 15 minute checks, Child # 5, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided licensee with an example sheet of the 15 minute checks and explained Safe Sleep regulations. Licensee stated that she will conducted the 15 minutes checks for all children under the age of 24 months. Licensee stated she will provide proof of the 15 minute checks for Child #5 to the department by plan of correction due date 7/19/2024.

Based on record review, the licensee did not comply with the section cited above in that LICENSEE, ASSISTANT #1, and ASSISTANT #2 did not have a current Mandated Reporter Certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that herself, Assistant #1, and Assistant #2 complete the AB1207 Mandated Reporter Training for Child Care Providers.

Based on record review, the licensee did not comply with the section cited above in that Licensee and Assistant #1 did not have a current CPR & FA, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she and Assistant #1 will complete the CPR &FA for child care and submit proof to the department by the plan of correction due date.

Based on record review, the licensee did not comply with the section cited above in that LPA observed that Child 1, Child 2, Child 3, Child 4, Child 5 did a not have a complete file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the files for Child 1, Child 2, Child 3, Child 4, and Child 5. Licensee will provide proof of corrections to the department by plan of correction due date of 7/26/2024.
