Based on observation, the licensee did not comply with the section cited above due to LPA observing that Child #1 had toys and objects on the play yard and Child #4 was wearing a bib while asleep in the play yard which posed a potential health, safety, and personal rights risk to children in care.
Resolution: Licensee removed all items from the paly yards including the bib from Child #4 during the inspection.
Based on observation, interview, record review, the licensee did not comply with the section cited above due to not having a copy of their assistant's immunization records for measles and pertussis which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, a copy of the immunization record will be submitted by POC due date.
