Based on record review, Licensee did not meet above requirement. LPA observed S1 LIC 503 blank and S2 to not have in file during inspection. This poses a potential health, safety, and personal rights risk to children in care.

Infant Needs and Services Plan (b)(2) Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20) This requirment is not met as evidenced by the facility not having the LIC 9227 for infants up to the age of 12 months on file.
