MANZO FAMILY CHILD CARE

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed a 2A10BC fire extinguisher in the facility with no service tag or proof of date of purchase which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of service tag or proof of date of purchase within one year for fire extinguisher.

Based on record review and interview, the licensee did not comply with the section cited above in one out of four children's records which were not complete which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of a completed record for Child 4 inlcuding forms LIC 700, LIC 627, LIC 282, LIC 995, immunization record, LIC 9227 and the 15 minute infant sleep log for Child 4.

Based on observation at 12:20pm, LPA observed backyard to have various hazards such as loose cinderblocks, gardening tools stored accessible, corners piled with ladder and various debris, and dog feces through out the grass area. LPA did not observe backyard being used during inspection. This poses a potential health, safety, and personal rights risk to children in care.

Resolution: Licensee states she will clean out hazards and provide photo proof to LPA by POC date via email/text.
