Based on observation, the licensee did not comply with the section cited above in 1 out of 1 fire extinghuishers in the home which is past the service date of 3/4/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have fire extinguisher serviced or will purchase a new one and provide the receipt with the date of purchase.
Based on record review and interview, the licensee did not comply with the section cited above in two out of two staff who have not completed the Mandated Reporter for Child Care Providers training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, proof of completion of Mandated Reporter for child care providers training will be submitted for herself and Staff 1 by the due date.
Based on LPA observation and interview at 11:20am, Licensee did not have mandated reporter training and interview confirms Licensee has not taken training. This is a potential health, safety, and personal rights risk to children in care.
Resolution: Licensee states she will take class and provide proof of completion to LPA via email/text.
