Based on record review, the licensee did not comply with the section cited above because her Adult daughter, Alina is living in the home and has not been criminally background checked, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Alina must be criminal background checked.

Based on observation, the licensee did not comply with the section cited above because sharp objects and cleaning products were observed in the kitchen area which was not closed off to children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Add a gate between the large room and the kitchen.

Based on record review, the licensee did not comply with the section cited above as the last fire drill was 01/02/2024, more than 6 months ago. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of an updated fire drill log with new entry.

Based on record review, the licensee did not comply with the section cited above in 2 out of 6 child file reviews, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Provide copy of immunization records and missing child files.

Based on observation and interview , the licensee did not comply with the section cited above infant was placed in swing from 9:20-10:09, at 10:09am LPA observed infant asleep in swing which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediatley moved infant from swing to the play yard. Stated she would not allow child to sleep in swing.

Based interview and record review, the licensee did not comply with the section cited above in 2 out of 12 children enrolled were missing documentation of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee to email documentation of immunizations for the two children.

Based on record review and interview the licensee did not comply with the section cited above in 4 out of 4 infant files were missing LIC 9227 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send copies via email of the LIC 9227 for the infants in care.
