Based on interview and employee record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. During employee records review, Licensing Program Analyst (LPA) observed that licensee's Mandated Reporter Training had expired as of March 2023 and Assistant #1 had never completed Mandated Reporter Training. LPA informed licensee that this training is a requirement and must be completed every two years for the safety of the children.
Resolution: Licensee agreed to complete Mandated Reporter Training as well as for Assistant #1. She will provide proof of completion by sending a copy of the certificates by email/text by the Plan of Correction due date, August 4, 2023.
