Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that Assistant #1 was missing tuberculosis (TB) test results. LPA also observed that Assistant #2 was missing proof of immunizations (Measles, Pertussis, Flu) as well as TB test results.

Resolution: Licensee agreed to obtain proof of immunizations (Measles/Pertussis/Flu) for Assistant #2 and proof of TB test results for Assistant #1 and Assistant #2 by the Plan of Correction due date, 11/21/25.

Based on observation and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed an uncleared adult in the home, providing care to daycare children.

Resolution: Licensee agreed to have the uncleared adult complete live scans and will provide proof of completion to the Community Care Licensing Office by the Plan of Correction due date, 10/29/24.

Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst observed that licensee was not documenting the 15 minute safe sleep checks for the infants in care on a consistent basis. LPA also observed two (2) sleeping infants covered by blankets.

Resolution: Licensee agreed to submit documentation that she is completing the 15 minute safe sleep log for the three (3) infants in care to the Community Care Licensing office by the Plan of Correction due date, 11/08/24.
