Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that child #1 was missing the emergency information card as well as all of the required licensing forms, including immunization records. Child #2 was missing immunization records and child #4 was also missing the emergency information card.
Resolution: Licensee agreed to obtain the missing licensing forms and/or immunization records for child #1, child #2, and child #4. She will send proof to the Community Care Licensing office by the Plan of Correction due date, 12/31/24.
