At 12:20 PM, LPA observed Adult 1(A1) providing care for children in care. A1 is not associated to facility. Upon further review, Licensee provided documents showing fingerprint cleerance request. LPA reviewed paperwork and logged onto Guardian which stated further documents needed. LPA provided letter to Licensee and A1 at time of inspection. This poses a potential health, safety, and personal rights risk to children in care at time of inspection.
Resolution: Licensee states she will email photo proof of submission of required forms by POC date.
At 1:30 PM, LPA conducted record review and did not observe LIC995 in any 5 out of 5 children's files at time of inspection. This poses a potential health, safety, and personal rights risk to children in care.
