Based on observation, interview and record review, the licensee did not comply with the section cited above as LPA observed multiple uncovered outlets accessible to children in care in the classrooms, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in LPAs did not observe tight fitted lids on trash cans which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in LPAs observed a staff member sitting in a chair, reading while she was supposed to be supervising children in care. During that time LPAs observed a child sitting in the opposite side of the room where staffs visual supervision was obstructed and the child was not being supervised which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in 6 out of 6 staff files that were reviewed, no staff have a current mandated reporter training. Per director she was unaware that the training needed to be redone every 2 years which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, record review, the licensee did not comply with the section cited above as 5 adults who are in direct contact with children at the facility did not have criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation and interview with director, the licensee did not comply with the section cited above in LPAs observed the sleeping mats with their sheets placed on top of one another. Per director the sheets are placed every morning and are stacked on top of each other without a barrier to ensure that they do not come into contact with one another which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, record review, the licensee did not comply with the section cited above as C1 has expired Epinephrine medication, which poses/posed a potential health, safety or personal rights risk to persons in care.
