Based on record review, the licensee did not comply with the section cited above in which a disaster drill log was not available at time of inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per license, she will conduct a disaster drill and document on the log provided by LPA and submit log as proof of correction to LPA via email by agreed upon date.
Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff did not have Mandated Reporter training certificates on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she had taken Mandated Reporter training. Licensee will locate training certificates for herself and staff and submit to LPA as proof of correction via email by agreed upon date.

Based onLPA observations, the licensee did not comply with the section cited above in that the Fire Extinguisher has not been serviced yearly-Last serviced on 3/23/17 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase a new fire exitngusher and provide a copy of the reciept by the POC due date.
