LPA met with Director / Alessandra Pinheiro and Center Coordinator / Rosaura Castillo who guided LPA on a tour of the facility. There were 34 children with 12 staff observed present in the infant program. During today's visit, LPA observed Staff 1 working at the facility prior to obtaining a criminal record clearance through Community Care Licensing. Based on paperwork obtained from the Director, Staff 1 obtained a background check through UCLA and began working at the facility. However, all individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Obtain a California clearance or a criminal record exemption as required by the Department. Facility failed to meet this requirement. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted with the Director and Appeals Rights provided. LPA met with Director / Alessandra Pinheiro and Center Coordinator / Rosaura Castillo who guided LPA on a tour of the facility. There were 34 children with 12 staff observed present in the infant program. During today's visit, LPA observed Staff 1 working at the facility prior to obtaining a criminal record clearance through Community Care Licensing. Based on paperwork obtained from the Director, Staff 1 obtained a background check through UCLA and began working at the facility. However, all individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Obtain a California clearance or a criminal record exemption as required by the Department. Facility failed to meet this requirement. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted with the Director and Appeals Rights provided.
