Based on observation, interview, record review, the licensee did not comply with the section cited above as an infant had a pacifier with an attachment while in their crib, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will read safe sleep regulation and write a declaration stating that they have read, understood and will follow safe sleep regulations moving forward. . Licensee will end signed declarartion to LPA by POC date of 12/05/25
Based on observation, interview, record review, the licensee did not comply with the section cited above as in licensee had blankets attached to the side of the cribs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will read safe sleep regulation and write a declaration stating that they have read, understood and will follow safe sleep regulations moving forward. Licensee will end signed declarartion to LPA by POC date of 12/05/25
Based on observation, the licensee did not comply with the section cited above, LPAs observed cleaning solutions that were made accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will remove all cleaning solutions from the children's bathroom and submit proof to LPA by POC date (01/12/2024)
Based on observation, the licensee did not comply with the section cited above LPAs observed 3 blankets placed on top of 3 cribs with no infants inside of the cribs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the blankets from the cribs and will be sending the blankets home when the infants are picked up.
