Based on observation, interview, and record review, the licensee did not comply with the section cited above in Licensee did not ensure assistant provided proof of the required immunizations (MMR, Tdap, and infliuenza vaccines) prior to being present in the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, assistant has a Doctor's appointment this month and will request proof of immunizations. Licensee stated that she will submit proof to LPA by POC due date.
