Based on record review, the licensee did not comply with the section cited above infant sleep charts completed last on 5/14/25 for 4 of 4 infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete infant sleep chart for all infants enrolled daily as infants sleep. Licensee will submit copies of infant sleep chart to the Department via email by 5/27/25 or next date of attendance.
Based on record review and interview with licensee, the licensee did not comply with the section cited above in 3 out of 3 children files did not have LIC 9227 Individual Infant Sleeping Plan, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have Authorized Represenative complete LIC 9227 and will place in infants file. Licensee will provide a copy to LPA through email or mail by POC date.
