Based on observation, interview, record review, the licensee did not comply with the section cited above in that Alyssa Dotson, is pending background clearance, was present and working in the facility which poses an immediate health, safety or personal rights risk to persons in care. A civil penalty of $100 is assessed on this date.
Resolution: The uncleared staff left the facility during the inspection. Per licensee, they will ensure the Alyssa Dotson will obtain the required background clearance prior to returnign to work at the facility.
Based on record review, the licensee did not comply with the section cited above in 2 out 3 staff did not have proof of immunization against pertussis and measles on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, proof of immunization for Staff will be sent to LPA by email by POC due date.
