Based on observation, the licensee did not comply with the section cited above in Licensee stopped documenting every 15 minutes once Child 3 (C3) and C4 turned 12 months, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she will continue to document every 15 minutes during nap time for all infants in care. Licensee understands that infants are 0-24 months.

Resolution: Per Licensee, assistant completed the Mandated Reporter Training; however, a copy of the certificate was not provided. Licensee will obtain a copy of the certificate and will submit proof to LPA by POC due date.
