Based on observation, the licensee did not comply with the section cited above, the toilet seat bowl cover is broken and covered with wrapped cloth material which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will repair toilet bowl cover by POC date and submit proof to LPAs.
Based on observation, the licensee did not comply with the section cited above. Per licensee she does not have a fire extinguisher at this time because she took it to get services, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a proof of serviced fire extinguisher to LPAs by POC date.
Based on record review, the licensee did not comply with the section cited above in, licensee stated the facility conducted a drill in February 2024, but there was no documentation to show that. The last documented disasted date was 09/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have a distaster drill by POC date and document drill on disaster drill log. Licensee will submit proof to LPAs by POC date.
