Based on observation,interview, record review, the licensee did not comply with the section cited above in which Avigail Hekmatjah and Bella Sebban, did not obtatin finger print clearence, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain fingerprint clearence for the two adults, and send LPA the Liv Scan by POC date.
Based on record review, the licensee did not comply with the section cited above in not obtaining current mandated reporter certification, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee informed LPA that they will obtain mandated reporter certfication, by POC date and email it to LPA Morales.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 childrens records did not have the LIC 995A, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will give parents the LIC 995A to sign, and will submit to LPA Morales via email.
