Based on observation, interview, the licensee did not comply with the section cited above in out of 1 carbon monoxide detector was not operable which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a new carbon monoxide detector and have installed in the home in order to ensure children in care are safe. Licensee will have new carbon monoxide detector on or before the plan of correction date. Licensee will email LPA proof of purchase and provide photograph of Carbon Monoxide installed in home.

Based on observation, interview the licensee did not comply with the section cited above in 2 out of 2 fire extinguishers were not purchased or serviced within the last 12 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: License will purchase a new fire extinguisher and show LPA proof of receipt via email or have the current ones in the home serviced and provide LPA with photograph of service tag.

Based on observation, interview and record review, the licensee did not comply with the section cited above in All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Licensee does not have documentation of fire and disaster drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will practice Fire and Disaster drill with children and send a copy of proof of practice to LPA.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 out of 2 people, both licensee's do not have current Mandated Reporter Training Certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's will provide current LPA with current Mandated Reporter Training Certificate via email on or before the plan of correction date.

Based on [(observation) (interview) the licensee did not comply with the section cited above in [3] out of [3] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will go on the website and book a time for the staff to take the mandated reporter training. Licensee stated that she will take a picture email to LPA.

Based on [(observation) (interview) , the licensee did not comply with the section cited above in 2 out of 2 (persons)] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will call her doctor to see how she will be able to retrieve her immunization record and the other staff will turnn immunization record to her take a picture and email to LPA.
