Based on observation, interview, the licensee did not comply with the section cited above in licensee obtained an infant swing at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide a declaration stating they will not use infant swings or any other equipment that can cause a potential danger and not permitted by the department. Licensee will provide declaration to LPA by POC date of 2/20/26
Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out of 3 staff did not obtain current mandated reporter training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that S2 and S3 complete childcare mandated reporter training and provider a copy to LPA by POC date of 3/06/26
Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff did not obtain proof that they have been immunized against pertussis which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that S2 is immunized against pertussis and provide proof to LPA by POC date of 3/18/26
