Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 infant did not have a 15 minute safe sleep log documented for LPA to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will start documenting safe sleep checks for infants newborns - 2 years old every 15 minutes and note labor of breathing and other observations. Licensee will send LPA sleep log for infants in care on POC date 3/28/2025.
