Based on observation and record review], the licensee did not comply with the section cited above in the uncleared adult, Monique Morin was observed to be providing care and supervision to children in care and does not have fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee asked uncleared adult Monique Morin to leave the facility and will ensure the uncleared adult is fingerprint cleared and associated to the facility prior to returning.
Based on observation, the licensee did not comply with the section cited above in the licensee had 15 children in care which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee had an uncleared adult Monique Morin who is also the nanny of the 15th child. Both the nanny/uncleared adult Monique Morin and the 15th child were asked to leave the facility. Licensee signed a declaration that she will remain in ratio at all times.
Based on record review, the licensee did not comply with the section cited above in the licensee does not have a current mandated reporter training as it expired on 06/26/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete mandated repoter training by POC date (08/09/2024)
Based on record review, the licensee did not comply with the section cited above in licensee's assistant does not immunization records on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain a copy if assistants immunizations by POC date (08/09/2024)
