Based on observation, the licensee did not comply with the section cited above in LPA observed child 1 (C1) and C8 did not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will submit proof of immunizations to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed 2 rusted desks in the back yard. LPA also observed wood stacked next to the storage, playground equipment was dirty (dirt/mud/spider webs) and did not have a cushioned material under the playgound equipment (slides) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will remove the 2 rusted desks and wood. Licensee will also maintain the playground equipment and will ensure that it is safe and clean prior to children use. According to the licensee, she will add a cushioned material under the slides.

Based on observation, the licensee did not comply with the section cited above in Licensee last conducted a disaster drill on 11/30/23 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will conduct a disaster drill and submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in Licensee Rada stated that Co-Licensee Estelita Ele has a job in a school cafeteria for the past 8 years which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will speak with Co-Licensee Ele regarding the requirement of absences cannot exceed 20% of the operation hours per day. Both licensee's will decide what will be best, will notify LPA, and will submit a written declaration by POC due date.

Based on observation, the licensee did not comply with the section cited above in Child 8 (C8) did not have a 15-minute infant log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will submit proof to LPA by POC due date.

Based on observation the licensee did not comply with the section cited above in LPA observed child 1 (C1), C2, C3, C4, and C6 are missing LIC 627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will submit proof of completed LIC 627 to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in Licensee Rada is caring for 5 infants all present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will disenroll 1 infant and will speak to the infant's parents. Licensee will submit a written declaration to LPA on how she will ensure to always maintain ratios by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in assistant is missing proof of TB, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, assistant will obtain proof of TB and will submit to LPA by POC due date.

Based on observation and interview, the licensee did not comply with the section cited above in licensee keeps the required form in the back day care room, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will move the parent board with all required forms in a visible area to parents.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Co-licensee is missing proof of influenza vaccine which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit proof of influenza vaccine to LPA by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in Co-licensee and assistant are missing proof of mandated reporter training certificates, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Co-licensee and assistant were in the process of completing the Mandated Reporter Training. Licensee will submit proof to LPA by POC due date.
