Based on observation, the licensee did not comply with the section cited above in LPA observed 2 rusted desks in the back yard. LPA also observed wood stacked next to the storage, playground equipment was dirty (dirt/mud/spider webs) and did not have a cushioned material under the playgound equipment (slides) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee Rada, she will remove the 2 rusted desks and wood. Licensee will also maintain the playground equipment and will ensure that it is safe and clean prior to children use. According to the licensee, she will add a cushioned material under the slides.

Resolution: Per licensee Rada, she will speak with Co-Licensee Ele regarding the requirement of absences cannot exceed 20% of the operation hours per day. Both licensee's will decide what will be best, will notify LPA, and will submit a written declaration by POC due date.
