Based on observation, interview, record review, the licensee did not comply with the section cited above in one uncleared adult in the home at the time of inspection.. Which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA White witnessed uncleared adult live immediately.

Based on record review, the licensee did not comply with the section cited above in licensee and assistant did not have proof of certification at the time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that proof of completion for Mandated Reporter Training for Licensee and Assistant will be given to LPA White on or before 10/30/2025

Based on record review, the licensee did not comply with the section cited above insix out of seven of the children's files did not have LIC 9150, LIC 282, LIC 627, LIC 700, LIC 995A. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that all required department documents and immunization records will be completed and available for inspection on or before 10/30/2025.

Based on observation, the licensee did not comply with the section cited above in cleaning products were observed in cabinet under the sink in bathroom that the children use, detergetns were on the floor of kitchen accessibel to children in care. under the sink, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the cleaning products and detergents inaccessble to children in care. Per licesnsee, a safety lock will be placed on the cabinet. Deficiency corrected during visit.

Based on observation, the licensee did not comply with the section cited above due an infant rocker was inside a play yard in the daycare room which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the infant rocker and placed in off limits bedroom. Deficiency Corrected during visit

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 infant under 2 years of age does not have a sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the sleep log fro Chidl #1 will be completed and a copy sent to the department via email.

Based on record review, the licensee did not comply with the section cited above in that the required Mandated Reporter Training (AB1207) has not been completed by Licensee or Assistant which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the mandated reporter training will be completed and the certificate will be provided to the department by email by POC due date.

Based on record review, the licensee did not comply with the section cited above in that proof of immunization was not available during this onspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licesee, they will provide copies for the the required immunization; MMR, and TDAP for Staff (Adult #2) to the department via email by POC due date.
