Based on observations and interview the licensee did not comply with the section cited above , licensee was not able to provide purchase receipt for the required fire extinguisher , which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase new fire extinguisher and will email picture of teh recipt to the LPA by POC date .
Based on observation,interview and record review, the licensee did not comply with the section cited above in 3 out of 3,licensee and 2 assistants had an expired Mandated Reporter Certificates , which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistants will take the training and email certificates to LPA by POC date .
