(b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedence This requirement was not met as evidence by record review. Lead test results revealed one water source had an action level exceedence. This is a potential risk to the health and safety of the children in care.

Based on LPA record review and observation, medication observed for C1 did not have label and not in original packaging. LPA record review of physician report does not indicate C1 has prescription for this medication. Site Supervisor states Authorized Rep did not provide prescription at time of inspection. This poses a potential health, safety, and personal rights risk to children in care.

(3) Prescription medications may be administered if all of the following conditions are met: (A) Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician.
