Based on observation play yard used for infants had no firm mattress and fitted sheets, the licensee did not comply with the section cited above in [1] out of [1] appropriate napping equipment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Picture of firm mattress and fitted sheets will be sent to LPA Calderon by poc due date.

Based on record review the 15 min slep log did not show 15 min time frame , LPA observed log for infants to have check marks per Licensee they indicate 15 min time checked and some were logged with start time and end time of naps, the licensee did not comply with the section cited above in [2] out of [2] 15 min log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: 15 min log sample was given to Licensees, Licensee will send copy of infant logs (2) to LPA Calderon by poc due date.

Based on record review, the licensee did not comply with the section cited above in one co-licensee missing mandated reporter certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per co-licensees, mandated reporter certificate to be completed and picture proof of certificate to be submitted via email.

Based on observation, the licensee did not comply with the section cited above, due to unsafe condiiton in back yard including: peeling and chipping paint on concrete steps, rusted metal poles, gate rail has sharp exposed edges, childrens table with exposed particle wood that is accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The backyard is separated from front yard by locked metal gate, licensee stated the children will not have access to the backyard until the hazardous items are removed or fixed. Licensee will provide proof of correction via email by POC due date.

Based on record review, the licensee did not comply with the section cited above in 1 out of 3 records reviewed has an incompleted LIC 627 Consent for emergency treatment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee the LIC 627 for Child #3 will be completed and proof of correction will be sent to LPA via email by POC due date.
