Based on observation, the licensee did not comply with the section cited above in LPA observed licensee place infant to sleep on their stomach which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee will place all infants to sleep on their backs at all times.

Based on observation, the licensee did not comply with the section cited above LPA observed barricade installed for stairs, but not utilized to barricade stairs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will use barricade for stairs at all times during business hours.

Based on record review, the licensee did not comply with the section cited above in 1 out of 4 files. LPA observed file to be missing completed LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide child's authorized representative with LIC9227 for completion and include in infant's file.

Based on observation, the licensee did not comply with the section cited above LPA observed beauty, hygiene products, and mouthwash in the bathroom in the shower, tub, counter, and medicine cabinet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all beauty, hygiene product and other items from the bathroom and make them inaccessible to children in care by placing them in locked cabinets, high shelves, or off limits areas.

Based on observation, the licensee did not comply with the section cited above in LPA observed medications and knives on kitchen counter/island which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed missing disaster drills/no drill documentation which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire/ disaster drill will chidren in care as needed every six months and document drills.

Based on observation, the licensee did not comply with the section cited above, LPA observed infant seated inside of a walker which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove prohibited item from home. Licensee will not use prohibited items in family daycare.

Based on observation, the licensee did not comply with the section cited above in LPA observed blankets loosely housed inside of the crib which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all loose items from the crib. Licensee will ensure cribs are free of loose items at all times.

Based on observation, the licensee did not comply with the section cited above in LPA observed a mobile attached to the side of the play yard which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove item from play yard. Licensee will ensure that no objects are attached to the side of the crib at all times.

Based on observation and record review, the licensee did not comply with the section cited above in LPA observed infant sleeping and when requesting infant sleep log, was advised by licensee that there is none which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will use an infant sleep chart to document as she supervises the infant sleeping.

Based on record review, the licensee did not comply with the section cited above in LPA reviewed childrens files and 3 of 4 files were missing immunization documentation which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will maintain children's files at all times, ensuring all documentation including immunization records are maintained in each child's file.
