Based on record review the licensee did not comply with the section cited above LPA observed more than 3 children files not having a copy of the LIC613A Personal Rights. which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation record review, the licensee did not comply with the section cited above LPA observed 2 children not signed in on sign in sheet during inspection. which poses/posed a potential health, safety or personal rights risk to persons in care.

Lead water testing samples taken on 06/05/2023 revealed that levels of lead were beyond the allowable amount deemed by the State of California. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care.

LPA observing that the facility does not have immunization records for Staff This poses a potential health and safety risk to children in care.

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Reporting Requirements The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). (1) Whenever a change in child care center director is reported, in addition to his/her name, the report shall include the following:The requirement was not met due to facility not notifiying CCLD of the passing of the Director on record and the hiring of a new qualified Director. This poses a potential health and safety risk to chidren in care.

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (B) Any injury to any child that requires medical treatment. Requirement not met due to CCLD receiving unusual incident report after the required time limit this casues a potiential health and risk to children in care.

Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion: (b)... child care provider, administrator or employee of a licensed chld care care facility shall complete the mandated reporter training provided... and shall complete renewal mandated reporter training every two years... This requirment is not met as evidenced by Staff #1 and #2 not having a copy in file of their training. This poses a potential health and safety risk to chidren in care.
