Based on observation, the licensee did not comply with the section cited above there were multiple occassions where Staff #1 was left out of ratio when Staff #2 exited the classroom which poses an immediate health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in 3 out of 5 teachers do not have the required mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in director nor staff were available to assist LPAs for inspection as director was not present and lead teacher needed to be in ratio which poses an immediate health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in 2 out of the 5 staff files that were reviewed did not have current immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.
