MODERN SCHOOLHOUSE, THE

Based on observation, the licensee did not comply with the section cited above in LPA observed staff #1 (S1), S3, S4, S5, S6, S7, S8 need to complete the mandated reporter training AB 1207 which poses/posed a potential health, safety or personal rights risk to persons in care.

Type B citation - potential risk if not corrected

Based on staff interviews, children's records review, and LPAs personal observation, it has been determined that there are 4 children under 18 months attending the toddler program. This poses an immediate health and safety risk to children in care.

Director stated that children were not disenrolled and continued to provide services to children under 18 months.

Type A citation - immediate risk to health, safety, or personal rights

Responsibility for Providing Care and Supervision : (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Licensee and staff were aware that C1 had a propensity to go to the sand box area and put sand in their mouth. Staff would remove C1 after consuming sand. Sand box area was not made inacessible and staff did not supervise C1 to prevent any further incidents.
