Based on observation, interview, and record review, the licensee did not comply with the section cited above as fire extinguisher provided in the home was pruchased more than one year ago, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase a new 2A-10BC fire extinguisher and provide proof of purchase to LPA P. Ochoa by POC date of 2/02/2025

Resolution: Licensee will have spouse obtain tuberculosis clearance and provide proof to LPA P. Ochoa by POC date 2/28/2025
Based on observations, the licensee did not comply with the section cited above . LPA observed open face floor heater accesible to the children which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will block the heater so children won't be able to touch it and email photos to the LPA by POC Date .

Resolution: Licensee and assistant will take the training and email certificates to the LPA by POC date .
Based on observation and record review, the licensee did not comply with the section cited above. LPA did not observe sleep log in the infant file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will start checking on infant every 15 minutes and log in the sleeping log and will email the copy to the LPA by POC date.
