Based on tour of the facility and licensee's statement, the licensee did not comply with the section cited above and 2A10BC fire extinguisher was not serviced as it is required which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will purchase a new 2A10BC fire extinguisher and a picture of receipt will be sent to LPA via email by 03/06/26.

Based on facility file review, the licensee did not comply with the section cited above and licensee did not have current facility roster for review during this inspection as it is required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have her current facility roster by 03/06/26 and a copy will be sent to LPA via email.

Based on 5 children file review and licensee statement, the licensee did not comply with the section cited above, per licensee infants sleep in the living room under an adult supervision all the time but she did not document every 15 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, effective today she will start to document infants breathing and position every 15 minutes and will document it. LPA provided a copy of Infant Sleep Nap Chart to licensee.

Based on licensee file review, the licensee did not comply with the section cited above and did not have required mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, she will complete the required mandated reporter training and a copy of certificate will be submitted to LPA. www.mandatedreporterca.com

Based on LPA's tour of the facility, the licensee did not comply with the section cited above. During this inspection the required 2A 10 BC Fire Extinguisher was available in the dining room but it has not been serviced annually as it is required which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will either buy a new Fire Extinguisher or service the one that she has and will submit a copy of receipt or service tag to LPA Eivazian by plan of correction due date 05/31/2022.

Based on LPA's file review, the licensee did not comply with the section cited above and she did not have required mandated reporter training certificate of file during this inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will complete the required mandated reporter training at www.mandatedreporterca.com website and a copy of certificate will be submitted to LPA Eivazian by plan of correction due date 05/31/2022.
