Resolution: Licensee will provide LPA Dowling with proof of child # 1 immunization record and proof of child # 2 having Measles, Mumps, Rubella (MMR) immunization on or before due date via email.
Based on record review and Licensee's interview statement the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Assistants will complete Mandated Reporter Training and submit proof of completion by POC date. Licensee will submit by email, fax or mail.
