Based on LPA record review and interview, 4 of 4 staff members had pediatric First Aid/CPR that expired on 01/2021; and per Director they have not taken updated training for this. This poses a potential health, safety, and personal rights risk to children in care at time of inspection.

Based on LPA record review, 2 of 4 staff members did not have proof of test of TB readily available at time of inspection. This poses a potential health, safety, and personal rights risk to children in care at time of inspection.

Based on LPA record review and interview, LPA did not observe current proof of completion of mandated reporter raining in 4 of 4 staff files (all expired on 02/14/2021); and per Director, they are in need of updating this training. This poses a potential health, safety, and personal rights risk to children in care at time of inspection.

Based on LPA observation and record review, the licensee did not comply with the section cited above in 4 out of 15 children present were not signed in or were signed in on incorrect date which poses/posed a potential health, safety or personal rights risk to persons in care.

Action Level Exceedance. This requirement was not met as evidence by: Based on record review the Licensee did not ensure water source at facility to meet lead requirements. Lead test results received showed that one water source has an action level exceedance of 18 ppb. This poses a potential health, safety, and personal rights risk to children in care.
