Based on record review and interview with licensee, the licensee did not comply with the section cited above licensee stated she has not completed the disastre drill as fire department stated they did not require drills which poses a potential health, safety or personal rights risk to persons in care. LPA informed the licensee that drills are required minimum every 6 months
Resolution: Licensee will complete and document disaster drill by POC date (01/29/2025).
Based on observation, the licensee did not comply with the section cited above LPA observed a baby walker in the living room which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's assistant/spouse removed the walker and placed in an inaccessible area and stated she will not use the walker any more.
Based on observation, the licensee did not comply with the section cited above LPA observed 2 blankets hanging over the playpen, no infant was in the playpen which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's assistant/spouse removed the blankets from the playpen immediately.

Based on observation, the licensee did not comply with the section cited above in LPA observed a 3 tier water fountain with water in the front yard of the home. LPA also observed that the fountain has a metal mesh gate that does not meet Title 22 Regulations which poses an immediate health, safety or personal rights risk to persons in care. LPA provided a copy of the regulation.
Resolution: Per Licensee, she will install the old gate that was placed in the past and that meets Title 22 Regulations. Licensee was reminded that the gate must be at least 5ft.
Based on observation, the licensee did not comply with the section cited above in LPA observed the kitchen in disarray. LPA observed the stove to be dirty with dried grease and food crumbs on the stove. LPA observed the air fyer dirty with food crumbs and a plate of chicken nuggets on top of the air fryer not properly stored. Kitchen counter was observed to have trash, a container with food, and storage food items not properly stored. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she usually cleans early morning or late afternoon. Licensee attempts to clean during nap time, however, it's not possible. Licensee will ensure food is stored properly and trash is discarded after every use. Licensee will submit proof to LPA by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in Licensee is not documenting every 15 minutes while infants sleep, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she will start documenting every 15 minutes during nap time for infants 0-24 months and will submit proof to LPA by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in Licensee's husband/assistant has an expired Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, husband/assistant will complete the Mandated Reporter Training and submit to LPA by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in Licensee's husband/assistant does not have proof of MMR vaccine, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, husband/assistant will obtain proof of MMR vaccine and will submit to LPA by POC due date
