Based on observation, record review, and interviews, the facility failed to comply with the license issued for age ranges 2 through entry into 1st grade by enrolling and accepting Child 1 prior to the child's second birthday, which poses an immediate health, safety, or personal rights risk to children in care.
Staff did not ensure that the children's Personal Rights were secured by refraining from hitting the children in care which poses an immediate risk to the health & safety of the children in care.
(d) All individuals subject to criminal record review shall, be fingerprinted and sign a Criminal Record Statement (LIC 508 [Rev. 1/03]) under penalty of perjury.
