Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 Staff (Licensee and S2 did not have a current Mandated Reporter Certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to send LPA a copy of Licensee's and S2's Mandated Reporter Certificate by the POC due date.
Based on Intrveview and record review, the licensee did not comply with the section cited above in that Staff #1 did not have an EMSA approved CPR/1st Aid which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the proof of completion of EMSA approved Pedriatric CPR/1st aid will be sent to LPA via email by POC due date.
